Everyone was able to get some sleep. Anna and Steve are doing well. Isaiah was quite lucid this morning, continually aware and trying to move and talk. We've done a lot of talking with him - encouraging and explaining. They decided to increase his sedation level rather than extubate at this time. He is now deeper into sleep.
The reasons that he remains on the vent are:
- the pressure in his abdomen reduces his lung capacity and ability to get full, oxygen-rich breaths
- if we extubate and Isaiah is not able to maintain his oxygen levels on his own, he would have to be re-intubated. For many patients the option of using something like a c-pap machine that aids breathing from outside your body is not an option for Isaiah due to concern of filling his abdomen with air.
- He has to demonstrate that he can keep his oxygen levels up in order to move towards removing the vent. This likely will not happen until there are more marked changes in the pressure in his abdomen.
We continue to give thanks that progress is being made every moment. His body is doing hard work that is slow and steady and being at rest is the best way to aid that process. You can continue to pray for wisdom for all the care teams involved (including his parents!) as they navigate this waiting process and make decisions on when to make larger changes. We also continue to pray for relief from the abdomen pressure - a huge spewing of gas would be celebrated!
